I agree to provide support and assistance for extra curricular activities both inside and outside the school as I am able.
Make myself available should there be a need for us to have a discussion about my child’s academic progress and/or behavior.
I understand that during the course of the school year, my child(ren), may be photographed on still pictures and/or video as part of the day to day school activities. This is in order to preserve memories and further advance the school in the community.
Examples of use: Brochures, school website, yearbook, Face book, newspapers and magazines.
I give my permission for PCA to photograph my child(ren) and use said image any and all examples above.
I grant permission to Pathway Christian Academy (PCA) and its officers, employees, representatives and/or agents for my child(ren) to ride in vehicles driven approved by the school to and from the playground for recess or on school-sanctioned field trips. I understand that my child(ren) will be safely secured in either a passenger safety system - as required by law - or with a standard seatbelt; and that the vehicle(s) license, inspection and liability insurance will be current. Driver(s) will possess a current driver license.
I hereby give consent for my student(s) to participate in field trips and / or athletic activities sponsored by Pathway Christian Academy (PCA). I understand that participation in a supervised physical education program has certain risks of injury. I hereby release, indemnify and hold harmless, PCA and its officers, employees, and agents from any liability, costs and damages resulting from my child’s participation in school-sponsored field trips and/or athletic activities. In the event the above named child becomes ill or injured while participating in a school-sponsored activity, I also give my consent for Pathway Christian Academy and its officers, employees, or agents to seek emergency treatment for my child, if necessary and as seems reasonable; and I agree to accept financial responsibility for the costs related to this emergency. In case of a life-threatening emergency, I authorize PCA and its representatives to consent to and obtain emergency medical treatment of my child in case of any illness or injury in connection with a school activity or school trip, such treatment to be administered by such physicians, other medical personnel, hospitals, and/or clinics as may be selected by PCA or its representative. I hereby assume responsibility for such professional services. It is understood that PCA assumes no responsibility in case an accident occurs.
Pathway Christian Academy shall have high spiritual and academic standards and shall include the development of the whole person spiritually, mentally, socially, physically, and emotionally. Emphasis is placed on learning about God and the truths of God's Word in relationship to mankind and the world through the academic process; recognizing that the way to God comes through personal faith in Jesus Christ, and Christian maturity comes by application of the truths of the Bible in all areas of life. Professing faith in Christ is not a prerequisite to enrollment – not for the student or parents. We believe in sharing our faith and not forcing it. We share our faith in Christ unashamedly, but we do so in a spirit of love toward the hearer. In fact, we view ourselves as sinners saved by Grace.
As a parent or guardian, I agree with the following statements:
I pledge my fullest cooperation to the school and administration. If a misunderstanding should arise, I will register the necessary comments and complaints only with a teacher, secretary, or principal.
As a school dedicated to Jesus Christ, there may be certain standards that may go beyond some of my personal attitudes and opinions as an individual; therefore, I willingly accept this position in my responsibility to uphold school standards, rules and regulations.
I further agree to authorize Pathway Christian Academy to employ such discipline as seems wise and expedient for my child. I also agree that I will cooperate and discipline my child in the home as needed. (NOTE***- Pathway does not practice corporal punishment.)
I agree to follow the terms and conditions regarding our financial obligation to the school. I also know I am liable for any damages to school property that my/our child is responsible for committing. Damage to school property, which includes textbooks, may cause your child's report card or other records to be withheld until damages are paid.
I understand that educational excursions away from the campus are a regular part of the total educational experience at Pathway Christian Academy. I give permission for my child to take part in these activities, or any school sponsored activity, educational or otherwise, away from the school premises. I understand I will be notified previous to the time when there is such an activity away from the school.
I absolve the school from liability to me or my child because of any injury incurred at school during any school sponsored activity, or school sponsored activity away from school, with the understanding that insurance will be maintained on my child during the school year.
I agree that Pathway Christian Academy seeks to teach moral and Christian principles. On occasion, the atmosphere or conduct within a particular home may be counter or in opposition to the biblical lifestyle the school teaches. This includes, but is not necessarily limited to, sexual immorality, illegal drug and substance abuse and/or inability to support the moral principles of the school. In such cases the school reserves the right - within its sole discretion - to refuse admission of an applicant or to discontinue enrollment of a student.
